PASSPORT NAME CHANGE:

 

People may require a name changed on their U.S. passport for various reasons including amending your married name or maiden name, or changing your passport for professional reasons.


As of September 26, 2005, the U.S. Passport Services Office will no longer amend valid U.S. passports for a legal change of name. Instead, you will need to apply for a replacement passport. To get started, please visit our Passport Renewal section for more information.
