CANCELLATION & REFUND POLICY - March 15, 2007
Disclaimer:
All references to Official Travel Documents, for purposes of this policy, shall be inclusive
of any fulfillment providers used for services obtained. Presently, those providers include
VitalChek for birth certificate fulfillment.
Official Travel Documents makes every effort to secure your travel documents
(certified birth certificate copies) as you request. However, it is important that when you receive
your documents, that you verify that all the information you require has been
obtained, including that the birth certificate is a certified copy with the registrar's raised,
embossed, impressed or multicolored seal. By engaging Official Travel Documents, you agree that the
sole remedy against Official Travel Documents for damages in connection with the failure to obtain
requested documents shall be the return of the service fee charged.
Official Travel Documents shall not be held responsible, nor accept any liability for the actions of
any providing agency in delaying or not issuing such
applications for any reason whatsoever, nor shall Official Travel Documents be held responsible
for expense and/or delay arising from or in connection with: (a) incomplete application or (b)
incorrectly or falsely completed application or (c) inaccurate or incomplete supporting documentation.
In the case of certified Birth Certificate copies, all of the above applies as well as (d)
non-entitlement of the requestor or (e) record not found by the providing agency of (f) wrong
certificate received or (g) incorrect information is shown on certificate. Any expense or
cost incurred by the client due to these delays or non-issuances shall not be the responsibility
of Official Travel Documents, nor shall any charges be levied upon Official Travel Documents.
Lost, damaged, or stolen travel documents
Should any requested travel document (certified birth certificate copy) or other item be
lost, misplaced, damaged, delayed or destroyed by any means, Official Travel Documents shall be
responsible only for the basic replacement cost of said item should the fault rest with Official
Travel Documents. This shall include original express courier shipping costs, only if initially
requested. Official Travel Documents is not responsible for items misplaced, damaged, delayed or
destroyed by a birth certificate provider agency or non-
traceable shipping service.
Government Fees
Government fees charged for birth certificate
services are subject to change without notice. Official Travel Documents reserves the right to
adjust the fees to obtain the documents for your trip by the specified date.
Government fees paid to Official Travel Documents are passed on to a government agency on your
behalf and are an absolute liability of the customer. Once the application has been submitted to
the appropriate government agency, these fees are non-refundable by Official Travel Documents.
Service and/or Processing Fees
Service and/or processing fees payable by the customer for services provided by Official Travel
Documents are exclusive of any government fees. Official Travel Documents
reserves the right to change the service or processing fee any time with or without prior
notice. Service or processing fees are considered non-refundable by Official Travel Documents
if the request has been provided to the appropriate provider agency or government office to fulfill.
If, however, the processing agent and/or his/her supervisor determine
that extenuating circumstances warrant a refund, Official Travel Documents reserves the right
to refund its service or processing fees on a case-by-case basis.
Shipping Fees
All shipping fees incurred are non-refundable by Official Travel Documents.
Cancellation Fee
If a birth certificate order is cancelled by the customer prior to being submitted to the
providing agency, the non-refundable processing fee will be considered the cancellation fee
and will be charged.
If birth certificate orders have been submitted to a providing agency
then orders cannot be cancelled from that point on, and all appropriate
service, government and shipping fees will be charged.
Abandoned Orders
If a birth certificate order is placed, and identification verification paperwork has been
requested but not received within 5 business days, the order is considered abandoned and
will be cancelled. If the order is cancelled due to non-receipt of requested information,
Official Travel Documents will charge the non-refundable processing fee as a cancellation fee.
